What is Endometriosis?

Endometriosis occurs when tissue similar to the lining of the uterus (endometrial tissue) is found outside the uterus, most commonly in the pelvis—around the ovaries, fallopian tubes, or on surrounding pelvic organs. In rare cases, this tissue may be located in the chest cavity or even within abdominal scars. This abnormal tissue responds to hormonal cycles, causing symptoms such as pain and infertility.

What Causes Endometriosis?

While the exact cause of endometriosis remains uncertain, several theories exist that work to explain how endometrial tissue ends up in abnormal locations:

  • Retrograde Menstruation (Sampson’s Theory): Menstrual fluid flows backward through the fallopian tubes into the pelvis, where the tissue implants and grows. This is the oldest and most widely accepted theory.
  • Vascular or Lymphatic Spread: Endometrial tissue spreads to distant sites through the blood or lymphatic systems.
  • Cell Transformation: Normal tissues transform into endometrial-like tissue.
  • Immune System Role: Current research explores how immune system dysfunction may contribute to the development of endometriosis. The immune system, which usually helps the body fight off infections, may not work properly in women with endometriosis. This dysfunction could allow the growth of endometrial-like tissue outside the uterus.

Symptoms of Endometriosis

Pain and Inflammation
Endometriosis can cause chronic pain and inflammation, often worsening during menstruation. The location of pain from endometriosis depends on where the abnormal tissue implants are, inflammation, and how inflammation might affect normal pelvic architecture. Some women will experience pain only with their menses, and others experience pain all of the time.

Endometriosis causes infertility in two ways. First, it causes an immunological response that promotes a hostile environment for the egg, sperm, and embryos in the pelvis. Secondly, endometriosis is an inflammatory condition that can destroy the normal pelvic architecture that facilitates egg pick-up by the fallopian tube. This can make it more difficult for an egg to be picked up by the fallopian tube, or for a fertilized egg to implant in the uterus.

Treating Endometriosis Pain

Treating endometriosis focuses on managing pain and reducing inflammation:

  • Medications: These are used to control pain and decrease inflammation. However, it’s important to note that many medications for endometriosis are not recommended for women attempting to conceive, as they may interfere with fertility efforts or fetal development.
  • Surgical Intervention: Surgery aims to remove endometrial tissue and alleviate pain. This approach is especially important when symptoms are severe or when conception is a priority. While surgery can be effective in reducing pain, it does not always resolve infertility caused by endometriosis. Nonetheless, it remains a valuable step in optimizing conditions for potential pregnancy.

  • Mild Endometriosis: Often responds well to controlled ovarian hyperstimulation with intrauterine insemination (IUI).
  • Severe Endometriosis: Achieves the highest success rates with in-vitro fertilization (IVF).
