Storage Options for Patients at ICRM

If you are a patient with stored sperm, eggs (oocytes), or embryos at ICRM, this page provides important information about managing your stored material, including options for continued storage, discontinuation, and donation.

Currently, donation options are limited:
  • Sperm: There is no option to donate frozen sperm samples. The choices are to continue storage or to discard.
  • Eggs and Embryos: You may choose to continue storage, discard, or, in some cases, donate anonymously.

All required forms for donation must be notarized if you are a U.S. resident. International patients should complete the forms and provide a copy of your passport—both partners, if applicable, must sign and submit the forms. Scanned and emailed copies are accepted; however, please keep the original documents in case they need to be resent.

Moving Your Stored Material to North American Cryobank

If you’d like to transfer your stored material to an off-site facility here in Boise, you can move it to the North American Cryobank (NACB). To do so, please:

Download and complete the Modulo di rilascio del trasferimento ICRM. This form must be notarized before returning it to ICRM.
Download the Pacchetto Cryobank nordamericano. Send this directly to NACB at the address provided on the form or scan and email it to Michelle@NAcryobank.com.

Please note:

All stored reproductive material frozen before 2017 has been moved to North American Cryobank due to space constraints.

Discontinuing Storage of Your Material

If you decide not to continue storage, you will need to complete the appropriate disposition form:
