Thank you for visiting the webpage for ICRM patients with frozen sperm, eggs (oocytes), and/or embryos stored at our clinic. Due to physical space constraints in our clinic, we will be moving stored reproductive material frozen prior to 2017 to an off-site storage facility located here in Boise, the North American Cryobank. Below is information and instructions on options for your stored sample(s).
Moving your stored material to North American Cryobank
If you decide that you would like to take advantage of moving your stored material to the North American Cryobank (NACB), please download and complete the following forms:
The ICRM Transfer Release Form will need to be notarized prior to returning the form to ICRM.
The North American Cryobank packet will need to be sent directly to North American Cryobank. The address for NACB is on the bottom of the form. You can also scan and email it to Michelle@NAcryobank.com.
Discontinuing storage of your frozen material
If you choose to NOT continue storage of your frozen material, you will need to fill out the appropriate disposition form.

Donating Frozen Material
There is no option to donate frozen sperm samples—the only choices are to continue storage or to discard. For oocytes and/or embryos, the options are to continue storage, to discard, and in some circumstances to donate anonymously. Forms DO need to be notarized if you are a U.S. resident. If you live internationally, please complete the form(s) and supply a copy of your passport (if you are a couple, both partners must sign form and send passport). Scanned and emailed forms are acceptable, but please retain the originals in case they need to be resent.

Please Note:
For your information, non-response to this request for consent to move your material will NOT keep your stored gametes and embryos at ICRM. They will be moved and you will be contacted by North American Cryobank for annual storage fees. All patients receiving this notification will be moved to NACB unless you directly reply to ICRM and request that your material not be moved to off-site storage.
